One possible reason: firewall blocks port 25 or server busy.....

Have you checked your settings in amavisd.conf like
$forward_method = 'smtp:[]:10025'; and your mail server setup?

amavisd send admin notifications normally over port 10025 (forward).

Can you provide some informations about your mail server and your
configuration (version, config etc)?
